Crisis Intervention

Crisis intervention is a time-limited intervention with a specific psychotherapeutic approach to immediately stabilize those in crisis.

Medication Management (Nurse NP and Psychiatrist)

Medications play a critical role in the treatment of various behavioral and mental health issues. The right medication can reduce or eliminate symptoms and significantly improve a patient’s quality of life.

Psychotherapy Assessment

A counseling or psychotherapy assessment is an initial session that is an important part of the therapeutic process and will help the therapist assess your circumstances.

Behavior Support Plan (BSP) (HCS and TXHML)

A "Behavior Support Plan" (BSP) is a plan that assists a member in building positive behaviors to replace or reduce a challenging/dangerous behavior. This plan may include teaching, improved communication, increasing relationships, and using clinical interventions.

Day Programs for Acute Needs (referrals)

Day programs for acute needs provide short-term, intensive treatment to individuals who require multidisciplinary therapy to stabilize acute psychiatric symptoms or prevent admission to a more restrictive setting. 

Mental Health Counseling

Mental health counselors work with their clients to establish goals and develop approaches to address emotional issues, develop coping and communication skills, improve self-esteem, promote changes in behavior, and strive for improved mental well-being.

Family Therapy

Family therapy is psychological counseling (psychotherapy) that can help family members improve communication and resolve conflicts. Family therapy is usually provided by a psychologist, clinical social worker, or licensed therapist.

HOW TO CHOOSE A PROVIDER 

·Here are the two vital truths you must know: 

1. Whether your slot Is just out or you are already receiving services in the community through a Provider, you have the right, at any time, to choose the Provider who will meet your needs in the community.

2. Every individual receiving care and services under the Texas Medicaid HCS waiver program has a SERVICE CO-ORDINATOR assigned to him/her. So, you have a Service Coordinator.

FAQ 1. MY SLOT IS JUST OUT. WHAT SHOULD I DO? The Service Coordinator who brings you this good news will allow you to choose a Provider to serve you. You might need help making that choice. You may be given a list of providers or Provider brochures or directed to a website, or an enlightened family friend may guide you. Once you choose your Provider, your Service Coordinator will respect your choice and contact the Provider on your behalf. The SC will initiate and conclude your placement in the community. 

FAQ 2: I AM ALREADY IN THE PROGRAM, BUT I WANT TO TRANSFER TO ANOTHER PROVIDER. WHAT SHOULD I DO? 

1. When your next Services Coordinator visits you, share your concerns and decisions with them. Your family can do that on your behalf. 

2. The Service Coordinator will ask you if you have any Provider in mind who can serve you better than the current one. If you already know where you want to go, tell the SC the new Provider's name. If you do not, request the SC to assist you in finding a good provider OR ask your Service Coordinator to call LIVINGMIND. 

3. In either case, the SC will discuss with your current Provider and contact your new Provider. The Service Coordinator will initiate and complete the transfer process.
